Missed the Outbound Immigration check

Hi All-

I came to US in April 2005. Recently I noticed that I do not have the indian immigration seal ( immigration check while leaving the country). My international flight was from mumbai to LA but I took the AI connecting flight from delhi to mumbai. I am not sure if I would had to get the immigration check at Delhi or Mumbai. Since my date of departure is not in their data base. Is anybody knows -

1. The process to get it fixed.
2. What are the documents needed to get it fixed.
3. Is there any implications while my restamping for US visa ?
